Summary

This article discusses the use of fractional flow reserve (FFR) with angiography for detecting significant coronary artery stenosis. FFR is the only functional index that has been validated against a true gold standard. All studies performed in a wide range of clinical and angiographic conditions found an FFR threshold of 0.75 to 0.80 to detect significant stenosis, with a sensitivity of 90% and specificity of 100% [Pijls NHJ et al. N Engl J Med 1996].
