By 2030, an aging population and other risk factors [Kones R. \u003Cem\u003EDrug Des Devel Ther\u003C\/em\u003E 2011] are expected to drive that figure to $276 billion [Heidenreich PA et al. \u003Cem\u003ECirculation\u003C\/em\u003E 2011].\u003C\/p\u003E\n         \u003Cp id=\u0022p-3\u0022\u003ERaul D. Santos, MD, PhD, University of S\u00e3o Paulo, S\u00e3o Paulo, Brazil, discussed the role of lipid management in CVD. The Statin Therapies for Elevated Lipid Levels Compared across Doses to Rosuvastatin [STELLAR] trial showed that across dose ranges, rosuvastatin decreased total cholesterol significantly more (p\u0026lt;0.001) than all comparators [Jones PH et al. \u003Cem\u003EAm J Cardiol\u003C\/em\u003E 2003].\u003C\/p\u003E\n         \u003Cp id=\u0022p-8\u0022\u003EData from the Cholesterol Treatment Trialists\u0027 (CTT) Collaboration [\u003Cem\u003ELancet\u003C\/em\u003E 2010] suggest that reduction of LDL-C by 2 to 3 mmol\/L can reduce risk of heart attack, revascularization, and ischemic stroke by about 40% to 50%. Law et al. [\u003Cem\u003EBMJ\u003C\/em\u003E 2003] demonstrated that statins can lower LDL-C concentration by an average of 1.8 mmol\/L, reducing the risk of ischemic heart disease events by 60% and stroke by 17%.\u003C\/p\u003E\n         \u003Cp id=\u0022p-9\u0022\u003ENon\u2013HDL-C is the second lipid target of therapy after LDL-C. The relationship between percent non\u2013HDL-C lowering and CHD reduction is approximately 1:1 [Robinson JG et al. \u003Cem\u003EJ Am Coll Cardiol\u003C\/em\u003E 2009].\u003C\/p\u003E\n      \u003C\/div\u003E\u003Cdiv class=\u0022section\u0022 id=\u0022sec-3\u0022\u003E\n         \u003Ch2 class=\u0022\u0022\u003EPotential Risks of Statins\u003C\/h2\u003E\n         \u003Cp id=\u0022p-10\u0022\u003EMyalgias (including infrequent cases of rhabdomyolysis) and elevation in transaminases have been reported for all statins. New data with simvastatin indicate that co-administration of medications that interfere with the metabolism of simvastatin by the cytochrome P450 enzyme increase the risk of rhabdomyolysis [Study of the Effectiveness of Additional Reductions in Cholesterol and Homocysteine (SEARCH) Collaborative Group. \u003Cem\u003ELancet\u003C\/em\u003E 2011]. These findings led to an advisory from the FDA to adjust downward the dose of simvastatin when used concomittantly with certain calcium channel blockers, ranolazine, and amiodarone, and to avoid the use of simvastatin 80 mg in most patients [\u003Ca href=\u0022http:\/\/www.fda.gov\/Drugs\/DrugSafety\/ucm256581.htm\u0022\u003Ehttp:\/\/www.fda.gov\/Drugs\/DrugSafety\/ucm256581.htm\u003C\/a\u003E].\u003C\/p\u003E\n         \u003Cp id=\u0022p-11\u0022\u003EAlthough epidemiologic evidence indicates an inverse association between LDL-C and incident cancer, statins are not associated with an increased risk of cancer [Alsheikh-Ali AA et al. \u003Cem\u003EJ Am Coll Cardiol\u003C\/em\u003E 2008]. These findings have been corroborated in the CTT Collaboration [\u003Cem\u003ELancet\u003C\/em\u003E 2010]. Thus the epidemiologic data appear to be confounded by other factors associated with cancer (eg, malnutrition) that cause low LDL-C.\u003C\/p\u003E\n         \u003Cp id=\u0022p-12\u0022\u003EStatin therapy is associated with a slightly increased risk of incident diabetes. Sattar et al. [\u003Cem\u003ELancet\u003C\/em\u003E 2010] found 1 extra case after treating 255 (95% CI, 150 to 852) patients with statins for 4 years. Preiss et al. [\u003Cem\u003EJAMA\u003C\/em\u003E 2011] identified 2 additional cases in an intensive-dose group per 1000 patient-years. Waters et al. [\u003Cem\u003EJ Am Coll Cardiol\u003C\/em\u003E 2011] found a slightly increased risk of new-onset type 2 diabetes with high-dose atorvastatin treatment compared with placebo in the Stroke Prevention by Aggressive Reduction in Cholesterol Levels [SPARCL] trial. Risk factors across 3 large randomized clinical trials included baseline fasting glucose level and features of metabolic syndrome.\u003C\/p\u003E\n         \u003Cp id=\u0022p-13\u0022\u003EBased on these findings, Dr. Santos concluded that there is a clear association between cholesterol levels and the risk of CHD death, that cholesterol lowering reduces the risk of cardiovascular events and mortality without increasing the risk of cancer, and that statin use is associated with an increase in the risk of type 2 diabetes. However, the absolute risk is small and is greater in those at higher risk of becoming diabetic at baseline.\u003C\/p\u003E\n      \u003C\/div\u003E\u003Cdiv class=\u0022section\u0022 id=\u0022sec-4\u0022\u003E\n         \u003Ch2 class=\u0022\u0022\u003E2012 Update on Intensive Lipid Lowering\u003C\/h2\u003E\n         \u003Cp id=\u0022p-14\u0022\u003ERobert P. Giugliano, MD, SM, Harvard Medical School, Boston, Massachusetts, USA, provided an update on intensive lipid lowering. His presentation included a rationale for it and a review on the efficacy of statins. He also discussed their use in selected populations, new safety concerns, and future therapies to reduce LDL-C.\u003C\/p\u003E\n         \u003Cp id=\u0022p-15\u0022\u003EA key question is \u003Cem\u003Ehow low should we go\u003C\/em\u003E? Data suggest that the incidence of cardiovascular events would approach zero if LDL-C were \u0026lt;60 mg\/dL in primary prevention and approximately 30 mg\/dL in secondary prevention [Hochholzer W \u0026amp; Giugliano RP. \u003Cem\u003ETher Adv Cardiovasc Dis\u003C\/em\u003E 2010].\u003C\/p\u003E\n         \u003Cp id=\u0022p-16\u0022\u003EEvidence indicates that such goals are not beyond the realm of possibility. In the CTT Collaboration [\u003Cem\u003ELancet\u003C\/em\u003E 2010], additional reductions in LDL-C safely produced further decreases in the incidence of heart attack, revascularization, and ischemic stroke with no evidence of any threshold within the cholesterol range studied. A corroborating meta-analysis of women in primary prevention trials showed a one-third reduction in risk of CV death [Mora S et al. \u003Cem\u003ECirculation\u003C\/em\u003E 2010].\u003C\/p\u003E\n         \u003Cp id=\u0022p-20\u0022\u003EIn patients undergoing hemodialysis, treatment with rosuvastatin lowered LDL-C levels but had no significant effect on the composite primary endpoint of death from cardiovascular causes, nonfatal MI, or nonfatal stroke [Fellstrom BC et al. \u003Cem\u003EN Engl J Med\u003C\/em\u003E 2009]. Findings were the same in an earlier atorvastatin study [Wanner C et al. \u003Cem\u003EN Engl J Med\u003C\/em\u003E 2005].\u003C\/p\u003E\n      \u003C\/div\u003E\u003Cdiv class=\u0022section\u0022 id=\u0022sec-6\u0022\u003E\n         \u003Ch2 class=\u0022\u0022\u003ENew Safety Concerns\u003C\/h2\u003E\n         \u003Cp id=\u0022p-21\u0022\u003EAn FDA safety alert (February 28, 2012) warned of reversible cognitive impairment with statin treatment. However, data from observational studies and clinical trials do not suggest that cognitive changes associated with statin use are common or lead to clinically significant cognitive decline [\u003Ca href=\u0022http:\/\/www.fda.gov\u0022\u003Ehttp:\/\/www.fda.gov\u003C\/a\u003E].\u003C\/p\u003E\n         \u003Cp id=\u0022p-22\u0022\u003EHsia et al. [\u003Cem\u003EJ Am Coll Cardiol\u003C\/em\u003E 2011] assessed the impact of LDL-C levels \u0026lt;50 mg\/dL on CV and adverse events in a population of apparently healthy adults treated with rosuvastatin. Outcomes showed that rates of myalgia, muscle weakness, neuropsychiatric conditions, cancer, and diabetes were not significantly different among treated individuals with and without LDL-C \u0026lt;50 mg\/dL; memory loss was not a significant side effect (p=0.041).\u003C\/p\u003E\n         \u003Cp id=\u0022p-23\u0022\u003EExtended follow-up in the Heart Protection Study (11.0 years; SD 0.6) demonstrated that more prolonged LDL-lowering statin treatment produced larger absolute reductions in vascular events. Even after the end of study treatment with simvastatin, benefits persisted for at least 5 years without any evidence of emerging hazards [Heart Protection Study Collaborative Group. \u003Cem\u003ELancet\u003C\/em\u003E 2011].\u003C\/p\u003E\n      \u003C\/div\u003E\u003Cdiv class=\u0022section\u0022 id=\u0022sec-7\u0022\u003E\n         \u003Ch2 class=\u0022\u0022\u003EFuture Therapies to Reduce LDL\u003C\/h2\u003E\n         \u003Cp id=\u0022p-24\u0022\u003EDual inhibition attacks cholesterol production (statin) and absorption (ezetimibe), leading to a reduction in hepatic cholesterol stores and increasing clearance of cholesterol from the blood. Winkler et al. [\u003Cem\u003EAtherosclerosis\u003C\/em\u003E 2011] found that ezetimibe alone and in combination with simvastatin significantly reduced the concentration of atherogenic small, dense LDL-C in patients with type 2 diabetes.\u003C\/p\u003E\n         \u003Cp id=\u0022p-25\u0022\u003ECholesterol ester transfer protein (CETP) inhibition is another promising approach. CETP is a plasma protein that catalyzes the transfer of cholesterol ester from HDL to apoB-containing lipoproteins (very LDL-C and LDL-C) in exchange for triglycerides. A major study on this treatment approach will conclude in 2017 [\u003Ca href=\u0022http:\/\/www.revealtrial.org\u0022\u003Ewww.revealtrial.org\u003C\/a\u003E].\u003C\/p\u003E\n         \u003Cp id=\u0022p-26\u0022\u003EBiologics also show potential therapeutic value. Proprotein convertase subtilisn\/kexin type 9 (PCSK9) plays an important role in degrading LDL-receptor (LDL-R). The resulting decrease in PCSK9 leads to an increase in LDL-R and lower LDL-C levels [Garber K. \u003Cem\u003ENat Biotech\u003C\/em\u003E 2012]. McKenney et al.
